Most referrals to other specialists will require you seeing or speaking with a GP, however, there are a number of services locally, listed below that you are able to make self referrals to.

Anxiety and panic attacks

Anxiety is a feeling of stress, panic or fear that can affect your everyday life physically and psychologically.

Patients aged 55+ can now self-refer to Specsavers, Scrivens, or IDC for audiology and wax removal.

  • Support neurodiverse/autistic individuals with managing symptoms of low to moderate emotional wellbeing and mental health difficulties.
  • Provide an environment where all aspects of autism are recognised, accepted and respected.
  • Offer parent, family and carer support through peer activities.
  • Provide resources and signpost to other community-based support, as appropriate.
  • Give particular support for transition-age young people, age 18–25 years.

Depression is a low mood that can last a long time or keep returning, affecting your everyday life.

Musculoskeletal (Physiotherapy)

Physiotherapy helps to restore movement and function when someone is affected by injury, illness or disability. It can also help to reduce your risk of injury or illness in the future.

Social Prescribing

We support patients who are experiencing non-medical challenges, which are in turn having an effect on their overall health and well-being. We help you explore what matters to you and link you in with services that may be better suited to your needs.

We cannot solve your problems for you, but together we can help you take a step in the right direction. We are able to support you over a series of 6 sessions.

Some examples of areas we can support with:

  • Employment
  • Financial issues
  • Accessing support as an unpaid carer
  • Dementia support
  • Bereavement
  • Linking you in with groups and courses to support your mental well-being
  • Issues with housing
  • Finding a purpose and joining local groups

Adult Social Services

If the person they are caring for has increasing care needs (e.g. struggles with washing/dressing, eating and drinking, mobilising around the home etc), they can request a ‘Needs Assessment’ by calling 01926 410410.

Health and Wellbeing Coach

We help patients to increase motivation levels and can support you to achieve your health and well-being goals. We know making changes to our lifestyles can be difficult, so we offer a person centred approach and help you to set small goals that will all add up to a positive effect overall!

We are able to support you over a series of 6 sessions. Some examples of things we can support you with:

  • Set small, attainable goals regarding healthy lifestyle changes
  • Keep you accountable
  • Support you to manage your weight, lower alcohol intake, increase physical activity, manage chronic pain in a holistic way and manage sleep better
  • Refer you to weight management programmes (providing you meet the eligibility criteria)
  • Support you to build healthy habits into your daily life.

Parent Carers

Defined as a ‘parent carer’ if you are over 18, living in Warwickshire and have parental responsibility for a child with a disability (as defined below):

  • They are between 0 and 18 years old and they live in Warwickshire.
  • Their daily life is affected by one (or more) of the following:
    • a hearing impairment
    • a visual impairment
    • a learning disability
    • a physical disability
    • a chronic or life-limiting physical illness
    • a significant communication disorder (including autism) which substantially and adversely affects their ability to take part in normal day to day activities.
